Just Launched! Practis Forms Bright Futures Toolkit
Bright Futures is a national health promotion and prevention initiative, led by the American Academy of Pediatrics. Under the Affordable Care Act most group health plans and insurers are required to cover, without patient cost sharing, the preventive health care services found in the Bright Futures/American Academy of Pediatrics Periodicity Schedule. Bright Futures will help you deliver these services and improve the quality of well-child care you provide.
The Bright Futures Toolkit provides forms, tools and parent education information for health care providers, patients, and families to complete before, during, and after well-child visits.
Practis is pleased to announce that we have partnered with the AAP to offer their Bright Futures Toolkit to physician practice members through Practis Forms, a HIPAA compliant online form builder tool. Discount pricing will be offered for AAP members.
Using Practis Forms, providers can log-in to view and download completed Bright Futures Toolkit questionnaires. This streamlines your workflow and allows practice staff to collect completed questionnaire data before parents arrive to your office.
